                                                                   Midway Virtual Airlines Agreement

    By filling out this application and submitting you are complying to the Midway Virtual Airlines agreement. 1.)Pilots must to stay active fly at least two flights a month. If unable to complete this requirement will result in the title of being inactive, after one month of being inactive you will be removed from the Midway Virtual Airlines Roster. 2.) All flights must be done online on the VATSIM network (NO EXCEPTIONS). 3.) All flights must be flown in 1x simulation. 4.)All PIREP flights must be done with the assigned equipment as seen on the flight table. 5.) Midway Virtual Airlines is based on the honor system as far as PIREP flights, if found submitting faulty flights the pilot will be immediately removed from the roster and will not be able to submit a pilot application again.
